Title: Management of Post –Traumatic open laryngeal Fracture – A case report

Abstract

Background: 
Laryngeal fracture is a rare and potentially fatal traumatic injury with an estimated incidence of 1 in 30,000 patients admitted to severe trauma centers. Due to rarity of this type of injury, the overall mortality rate is 17.9%. Laryngeal trauma is usually seen in multiple-trauma patients, and laryngeal fractures are often undiagnosed or poorly managed leading to significant problems with airway patency, voice production, and swallowing. This paper reports a case of a 28-year male patient admitted to the emergency room after a motor cycle accident with a truck. The patient presented with lacerated open neck wound with multiple small lacerated wounds of the skin over the neck and chin. The computed tomography (CT) revealed a severe burst injury of the anterior larynx with fractured thyroid laminae, and larynx is exposed to the external surface. 
Method: 
This paper presents case study method of investigation going through patient medical records, medical and surgical intervention, and patient feedback. 
Results:
Early intervention with an open reduction and internal fixation of the thyroid cartilage helped to restore the near-normal anatomy of the larynx. Post-op, he had no swallowing or breathing problem except hoarseness of voice which also improved to reasonably good voice 6 months after surgery. 
Conclusion: 
Early diagnosis of the laryngeal injury is indispensable, and the most important goal in management is at first to secure and reconstruct the airway. Once this has been achieved, the long-term goal of treatment is to restore the voice and swallowing mechanism. Because of the rarity of this injury, physician may be not aware of its existence, leading to a late diagnosis of this entity.

Biography

Rajkumari Khatri (Sachdeva) has completed her master’s degree in Otorhinolaryngology from DAVV university, Indore, India in 1994 with Gold Medal. She has passed  her MRCSI from The Royal college of surgeons, UK in 2006.  
She had done Post PG surgical training at various recognized institutions at Mumbai, India. Since 1998, she has been doing private practice in ENT &Head and Neck Oncology. At present she is working as Laser Consultant at India Institute of Head & Neck Oncology. 
She had been awarded Life Time Achievement Award, Jewel of India Award, National Mahila Ratan Award for the achievement in the field of medical education. 
Apart from medical field, she has been working actively as Yoga prana Vidya Researcher, for studying the effect energy on the human body. 
To her credit, she had 10 research publications in journal of National & international repute. 
Recently she is doing research work on the role of PBM laser on oral mucositis in Post-CT & RT Cancer patients.
